SUPPORTED LIVING
We provide a Person centred and activity-based care and support to service users living in their homes in order to promote their choice and Independence;
The Support Includes:

Management of Tenancy.

Educational, training and work-based support that is exploring of volunteering and paid work opportunities.

Support with personal care and welfare.

Promotion of life skills such as meal preparation, gardening, budgeting and money management support, cleaning and management of their environment.

In-door and out-door activities.

Linking service users with the community health care and other mental health services.

Travel training and going out in the community

Emotional and behaviour management support.

Support with shopping.

Administering of medication.

Registering with local Health Care Professionals such as: Dentist, Gp, Optician and Mental health team,

Escort to health care professionals’ appointments.
-
Respite services in order to give carers a break
